Amazing.

In any other case, such a report from a member of the board should have led
to the liquidation of that organization. Or at least large scale
restructuring / reform.

I hope Kieren will take in these recommendations.

Why?

Let me point out one such written evaluation of ICANN's performance which
also contains several concrete recommendations.  It is an official ICANN
communication from a sitting board member of ICANN to the entire board of
directors delivered during a public meeting of that board. 
ICANN has never bothered to publish it despite a routine practice of
publishing similar materials.
